The most important difference between the bones of children and adults is the presence of growth plates. These are plate-shaped areas of cartilage that are usually located at both ends of the long bones and are responsible for the longitudinal growth of the bones. The growth plate is the weakest part of the bone, and when a blow strikes a child’s limb, the growth plate is often the first site to be injured. Growth plate injuries following trauma usually occur as separation of the growth plate from the bone or as the fracture line passing through it. These injuries are called growth plate fractures. ?Flexibility of children’s bones Flexibility is another important characteristic of children’s bones. When a child’s bone is subjected to a blow, it bends and straightens like a spring and does not break. Compared with adults, children’s bones can withstand greater forces without breaking because of their flexibility. Due to this high flexibility, two types of deformity occur in children’s bones following trauma that are rare in adults. Greenstick fracture: in this type of fracture, the bone behaves like a wet tree branch in response to the force applied to it. It does not break completely; rather, the two fragments remain connected while becoming deformed at the site of impact. ?Looseness of the periosteum’s attachment to the bone The periosteum, or periosteum membrane, is a cloth-like membrane or layer that covers and is attached to the bones of the body. In children, this periosteal layer is loosely attached to the diaphysis (the middle part of the long bones), and following a bone fracture, blood from the fracture rapidly collects between the periosteum and the bone, easily separating them from each other. This blood rapidly turns into bone callus. For this reason, bone callus is present in fractures in children, even in nondisplaced fractures, and is larger than in adults.
